Although their time with us may be brief, most of our patients/residents and their families feel that their days with Avalon Hospice are some of the most important they've ever spent.
Many choose to make donations after a rewarding experience with our group, while others who have not yet experienced hospice get involved based on their strong belief in our mission.
For those who wish to give back to our organization through a monetary donation, there are several options available. Please contact our agency for more information.
Memorial Contributions
If you would like to make a contribution to Avalon Hospice in memory of someone
who has died, you may mail it to us directly, or click here to make your
contribution electronically.
When making a memorial contribution, please tell us:
Name of the Deceased
Your Name
Your Address
Your Daytime Phone
Name and Address of Deceased Person’s Family Member (so that we can notify them
of the gift you make in memory of their loved one)
Please be sure to write legibly!

Avalon Hospice will acknowledge receipt of your gift within one business week
after receiving it; the family member of the deceased, whom you have asked to be
notified of your gift, will be contacted at the same time. The amount of your
gift will NOT be shared with the family member.
