Checks should be made payable to Avalon Hospice and mailed to 2360 Stone Bridge Drive, Flint, MI 48532. Walk-in contributions are also accepted at the Avalon Hospice office. May I make a contribution to a specific program or service (e.g. dementia care, charity care, Healing Hearts Youth Grief Camp, Beautification Garden, or Chronic Care)? Yes, but please indicate the program or service to which the contribution is to be applied in the memo portion of your check. Call us to discuss further details on specific programs and how your contribution may help. How do I make a memorial contribution? Please write the name of the deceased person in the memo portion of the check. May I use a credit card for my donation? Yes, Avalon Hospice accepts MasterCard and Visa. Please supply your card type, number and expiration date. May I make a contribution directly to Avalon Hospice staff members? No. Please don’t. All donations should be made to Avalon Hospice. Staff members are not allowed to accept compensation from patients, or the families or friends of patients for their services. Will the family be notified? Yes. We send a letter to the designated person, which includes a list of the names and addresses of those who have made a contribution, in memory of a loved one. The donation amounts are not divulged. Do I need to supply the name and addresses of the family member to be notified on my donation? That depends. If the contribution is in memory of a patient cared for by Avalon Hospice, the answer is “no.” We’ll have the family information on file.
